1. Strip it back to three fields Your initial goal is simply to start a conversation, not to run a full background check. Reduce your form to just three fields:

Email Address
"What is your biggest current challenge?" (Make this a text box). That is all you need to qualify a lead and begin an intelligent dialogue.
2. Never use the word "Submit" "Submit" is a cold, administrative word that subconsciously implies work or commitment. Change your button text to an action-oriented, high-value phrase. Use something like "Let's Talk," "Get Your Free Assessment," or "Send Enquiry."
3. The Mobile Check Open your phone right now and try to fill in your own contact form. If you have to zoom in to tap a box, or the keyboard covers the submit button, you are losing leads. Make sure it works flawlessly on a mobile screen.
